NRG INSPECTION REPORT NOS. 50-280/91-24 AND 50-281/91-24
This refers to the inspection conducted by Mr. M. W. Branch of this office on
August 2-26, 1991.
The inspection included a review of activities authorized
for your Surry facility.
At the conclusion of the inspection, the findings
were discussed with those members of your staff identified in the enclosed
inspection report .
Areas examined during the inspectidn are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
In addition, two apparent violations were identified and are being considered
for escalated enforcement action in accordance with the
11 General Statement of
Policy and Procedure for NRC enforcement Actions
11 (Enforcement Policy), 10 CFR
Part 2, Appendix C (1991).
The first apparent violation pertains to operation
of Unit 1 and Unit 2 with the common No. 3 emergency diesel generator
inoperable from May 9, 1991, to August 2, 1991.
The second apparent violation
pertains to the operation of both units in a specific electrical configuration
which resulted in inoperable high head safety injection pumps.
Accordingly, no
Notice of Violation is presently being issued for these inspection findings.
Please be advised that the characterization of the apparent violation described
in the enclosed inspection report may change as a result of further NRG review.
An enforcement conference to discuss these apparent violations has been
scheduled for September 17, 1991.
The purposes of this conference are to
discuss the apparent violations, their causes and safety significance; to
provide you the opportunity to point out any errors in our inspection report;
to provide an opportunity for you to present your proposed corrective actions;
and to discuss any other information that will help us determine the
appropriate enforcement action in accordance with the Enforcement Policy.
In
particular, we expect you to address .the safety significance of these two
problems occurring toget~er dufing the May to August, 1991 timeframe.
