NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-280/89-24 AND 50-281/89-24)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by

W.

E. Holland, L. E. Nicholson, and J. W. York during the period July 30

through September 2, 1989.

This inspection included a review of activities

authorized for your Surry facility.

The report documenting this inspection was

sent to you by letter dated September 28, 1989.

The report described two

examples of failure to comply with radiation protection rules and three

examples of failure to preclude repetition of conditions adverse to quality.

Accordingly, these concerns were discussed in an Enforcement Conference held on

October 17, 1989.

The letter summarizing this conference was sent to you on

November 12, 1989.

Violation A in the enclosed Notice of Violation involved two incidents where

radiological control requirements were not followed.

We are concerned that

although corporate and site management as well as the health physics staff are

sensitive to the necessity to adhere to health physics procedures, your

radiation workers are not yet attuned to your new level of expectation in this

area.

You must assure that problems discussed in this violation are minimized

to the greatest extent possible and that workers are adequately trained in

radiological control activities and recognize and accept personal responsi-

bility for their actions.

Violation Bin the enclosed Notice of Violation involved three incidents where

previous corrective action failed to preclude recurrence of problems associated

with maintenance activities.

In all three examples, corrective actions involved

issuance of a standing order or procedure with little or no work.er-level

training, management emphasis, or followup on adequacy.

Failure to incorporate

expectations at levels below site management was also a contribution to

Violation B.

We are specifically concerned over your failure to fully evaluate

the effectiveness of the corrective action in these cases, especially as it

reflects on your corporate philosophy on assuring thorough corrective action.

Based on our discussion during the October 17 Enforcement Conference, we

believe that you understand that corrective actions must be both comprehensive

and long-term and must be periodically evaluated for effectiveness.
