NRC INSPECTION REPORT NOS. 50-280/89-17 AND 50-281/89-17

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by

W. E. Holland; L. E. Nicholson, and J. W. York on April 30 through June 3,

1989.

The inspection included a review of activities authorized for your Surry

facility.

At the conclusion of the inspection, the findings were discussed

with those members of your staff identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

The inspection findings indicate that certain activities appeared to violate

NRC requirements.

A Notice of Violation (NOV) is not being issued in this

case, in that the substance of the violation discussed in the enclosed report

is essentially identical to Violation A in the Notice of Violation issued with

North Anna NRC Inspection Report 50-338,339/89-14 on June 29, 1989.

Your

response to this NOV will also address the violation discussed in the enclosed

report ..

As discussed in paragraph 8, we have a particular concern over the results of

our inspection of your offsite independent review group.

We acknowledge that

you are.in the process of increasing the staffing in this area in order to more

effectively accomplish the~e requirements; however, independent of this effort,

you. and your staff must ensure that Technical Specifications are complied with

during this important process.

We are not requesting that you respond to

this management support concern in that you have already been requested to do

so in North Anna Inspection Report 50-338,339/89-14.

In addition, we also note that an additional example of past problems with

regards to corrective action and root cause evaluation was identified with

regards to an inadvertent start of one of your emergency diesel generators.

We will continue to closely monitor programmatic enhancements in this area .
