SUBJECT:
(NRC INSPECTION REPORT NOS. 50-280/88-30 AND 50-281/88-30)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted
by N. Economos on August 1-5, 1988.
The inspection included a review of
activities authorized for your Surry facility.
At the conclusion of the
inspection, the findings *were discussed with those members of your staff
identified in the enclosed inspection report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedure!
and representative records, interviews with personnel, and observation of
activities in progress.
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The violation, references to pertinent requirements, and
elements to be included in your response are described in the enclosed Notic(
of Violation.
This finding points to certain potential weaknesses in**the performance of
inservice testing of pumps and valves as required by ASME Code Section XI, ar1
a 1 ack of effective quality assurance to ctssure corrective acti.on in a timel~
manner.
This was evidenced by the fact that failure to test the valve went
undetected since the 1985 refueling outage; that following identification vi,
the NRC audit, a deviation report issued in February 1988 was not reviewed a:
required by SNSOC; opportunities to test the valve since the finding were
ignored; and no effort was made to evaluate the valve's capabilfty to perfor1
its.,intended function until such an evaluation was specifically requested *b}
the inspector.

Therefore, your response should address -corrective actions which will be* takE
to identify other components/valves in the prpgram which may also not have
been tested and to enhance measures to assure the timely implementation of
administrative controls and quality assura*nce requirements.
Also provide
assurance that the valve in question is capable of performing its intended
function under design basis accident conditions .
