HUMAN PERFORMANCE STUDY REPORT - NORTH ANNA 2, 4/16/93 I have prov*ided a copy of the subject report directly ta you for information in addition to the normal distribution ta members of your staff because of two issues identified by the report.
- 1.
This event represents an additional instance in which operators inappropriately bypassed an £SF, the Auxiliary Feedwater System, after a valid initiation during the response to an event.
The auxiliary feed system was inoperable for 18 minutes until the discrepancy was identified and the equipment was restarted to operability.
- 2.
A man-machine interface weakness was noted in which the operator at the i
.J controls needs a spotter at important recorders to provide readings.

<. one case this l!d ta...a flow adjustment to the wrong steam generator due J to a con111un1 cat 1 on error.
The first issue is further evidence of a THI lesson that has faded despite an Information Notice last year.
The second 1ssue comes at a t1me that V1rg1nia Power has determined that earlier control room man-machine upgrades at Surry are no longer necessary.
I have asked my staff to check with the PM of Surry and North Anna regarding this particular HF issue.
