Division of Reactor Controls and Human Factors Herbert N. Berkow, Director Project Dfrectorate II-2 Division of Reactor Projects~ I/II Office of Nuclear Reactor Regulation NORTH ANNA POWER STATION, UNIT 2, SAFETY FEATURES BYPASS (APRIL 16, 1993) (TAC NO. M87203)
On April 16, 1993, a turbine trip/reactor trip occurred from 100% power.
The main feedwater pumps continued to operate and the auxilary feedwater (AFW) pumps started automatically when the steam generator level dropped.
A reactor operator (RO) then secured the AFW pumps to prevent overcooling.
However, the RO secureid the AFW by placing all AFW puinps in pull-to-lock (PTL) rather than in the "auto position. Therefore, the AFW would not have started automatically if called upon by safety actuation.
The AFW system was inoperable for 18 minutes until an SRO noticed the PTL position *and the RO returned the AFW switches to their normal positions.
As a result of this event, AEOD sent a team to North Anna to assess the human factors aspects and issued a Human Performance Study Report on April. 18, 1993.
In a memorandum (attached) from E. Jordan, Director, AEOD, to T. F. Murley, Director, NRR, and S.D. Ebnet~r,*Regional Administrator, RII, dated July 2, 1992, two human factors issues were identified which require your attention.
We request that you consider the two issues as identified in the July 2, 1993
-memo and provide your evaluation and conclusions by August 31, 1993.

HUMAN PERFORMANCE STUOY REPORT - NORTH ANNA 2, 4/16/93 I hc1ve provided a copy of the subject report directly to you for information in ciddition to the normal distribution to members of your staff because of two issues identified by the report.
- 1.
This event represents an additional instance in which operators inappropriately bypassed an ESF, the Auxiliary Feedwater System, after a valid initiation during the response to an event.
The auxiliary feed system was inoperable for 18 minutes until the discrepancy was identified and the equipment was restarted to operability.

A man-machine interface weakness was noted in which the operator at the i
- controls needs a spotter at important recorders to provide readings.

The first issue is further evidence of a TMI lesson that has faded desp1te an Information Notice la.st year.
The second issue comes at a t1me that Vfrg1nia Pow1~r has determrned that earlier contro 1 room man-machine upgrades at Surry are no longer necessary.
I have asked my staff to check with the PM of Surry and North Anna regarding,this particular HF issue.
