NRC INSPECTION REPORT NOS. 50-280/94-02 AND 50-281/94-02

. This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
. Mr. M. Branch of this office on January 2 through February 5, 1994.
The
inspection included a review of activities authorized for your Surry facility.
At the conclusion of the inspection, the findin~s.were discussed with those
members of your staff identified in the enclosed report.
Areas examined during the inspectiqn are identified in the report. Within
. these areas, the inspection consisted ~f selective examinations of procedures
and representative records, inter~iews with personnel, and observation of
activities in progress.
The enclosed inspection report identifies activities that violated NRC
requirements that will not be subject to enforcement action because the
licensee's efforts in correcting the violation meet the criteria specified in
Section VII.B of the Enforcement Policy.
During the inspection period, two Unresolved Items were identified.
One
- involved an unexpected de~rease in the Unit I.reactor vessel indicated water
level while the unit was shutdown and depressurized, paragraph 3.i. A similar
ev~nt ~lso occurred during the previous Unit 1 refueling outage.
We
- understand that your evaluation concluded the most probable cause of the
events to be restrictions in the reactor head vent piping; however, testing
- performed to date has been unable to verify this conclusion.
We are concerned
about the reliability of the reactor vessel standpipe level indication because
this instrumentation would be relied upon during mid~loop operations to
prevent a loss of shutdown cooling.
The second Unresolved Item involved a hydrogen gas burn inside the pressurizer
on February 3, 1994, for which the root cause evaluation has *not been
complet~d, paragraph 3.j.
We are concerned about this event because of the
potential damage to safety-related equipment.
In order to resolve these two issues, please forward to this offi~e, within 30
davs of the receipt of this letter; what additional actions ybu plan to take

to preverit future reactor vessel water level indication probl~ms and the
results and any corrective actions you plan to take as a result of your
hydrogen burn root cause evaluation.
In accordance with 10 CFR 2.790 of the NRC's "Rules bf Practice", a copy of
- this letter and its enclosure will be placed.in the NRC Public Document Room.
The response directed by this letter is not subject to the clearance
procedures of the Office of Management and Budget as required by the Paperwork
Reduction Act of 1980, Pub. L. No.96-511.
