The EDG was invnediately restarted and its operability verified.

Emergency start capability not impaired by a possibly insuffi-cient fan cooling system.

Surveillance of diesel generator batteries was late. Howeve~e:

emergency capability was not impaired.

Cause of failure was unknown.

Diesel was started and loaded short!

after the diesel trip and found to operate satisfactorily.

Each diesel was taken out-of-service to complete maintenance.

LER 78-125/IT-O Description of the Event Seismically Deficient Emergency Diesel Generator Battery Racks Vepco operating personnel requested the Architect Engineer to compare the emergency diesel generator battery rack seismic design drawings to the manufacturers drawings used to install the racks.

The Architect Engineer discovered that the wall mounts, the side rail supports and the bottom support rails differed from those specified by the *seismic design drawings.

Further investigation revealed that a request to modify the above mentioned components had been issued during construction but had not been implemented.

Seismic calculations showed the wall mounts and side rail supports would yield during a design basis earthquake (DBE).

Preliminary seismic calculations indicate that the bottom rails will not yield during a DBE.

ThJs event is reportable pursuant to Specification 6.9.1.8.i.

Modifications made to the battery racks to correct seismic deficiencies required alternate removal of the lJ and lH emergency diesel generators from service.

One emergency diesel generator remained operable at all times.

This event is reportable pursuant to T.S. 3.8.l.l and Specification 6.9.1.9.b.

Probable Conseouences of the Occurrence If a design basis earthquake occurred in the proximity of the North Anna Power Station, the e~ergency diesel generator racks may fail. Failure of the

. battery racks, which*resulted in battery failure, may cause the emergency diesel generators to become inoperable.

The station would rely on nonnal station electric power or offsite electric power.

Removal of the lH and lJ emergency diesel generators was perfonned in accordance with T.S. Action Statement 3.8.1.l.a.

No adverse consequences are expected.

Cause of Occurrence During field installation of the emergency diesel generator battery racks requested modifications were not made.

The lJ and lH emergency diesel generators were removed from service to make modifications to the battery racks to correct seismic deficiencies.

Immediate Corrective Action Seismically approved wall mounts were installed. ihe battery racks were inspected by representatives of the vendor and Architect Enginee~.

The operability of the "2H" diesel generator was not verified within one hour as required by the ACTION statement of T.S.

Probable Consequences of Occurrence The health and safety of the public were not affected since both offsite sources were available and the "2H" diesel was still operable.

In addition, the "2J" diesel generator shutdown relays were reset and the control switch placed in "Manual-Remote".

Since it is believed that "2J~ diesel trip was due to high crankcase pressure, an emergency start signal would have started the diesel and high crankcase presJure would not have caused a trip.

.Cause of Event The 2J" diesel generator trip was apparently due to high crankcase pressure since the trip occurred approximately 20 seconds after the start signal and the diesel had reached ~00 RPM in less than 10 seconds.

Since the "2J" diesel generator tripped, personnel referred to the ACTION statement of T.S. 3.8.1.1 which requires testing the frequency and voltage response of the other diesel generator.

However no procedures were written which included [nstructions for connecting test equipment necessary for verifying frequency and voltage response as required by T.S. 4.8.1.1.2.a.4.

Greater than one hour passed before procedure changes required for testing could be written and approved.

In addition, the ~aintenance Operating Procedure, 2-MOP-6.90 was in error in that it did not contain requirements fQr testing frequency and voltage response as required by T.S. 4.8.1.1.2.a.4.

Immediate Corrective Action The 2J" diesel g*enerator shutdown relays were reset and the contrcl s.itch placed in "Manual-Remote".

The offsite power breaker alignment was verified as per T.S. 4.8.1.1.1.a at 1130 on 5-17-SO.

At the time of removal, the snubber (a Unit II snubber on a Unit II system) was listed in both Unit's Technical Specifications.

For Unit II in Mode 5 the removal of this snubber was permitted but its removal was not permitted by its inclusion in Unit I technical specifications.

This is reportable pursuant to T.S. 6.9.1.9.b.

Probable Consequences of Occurrence Since the snubber removed was installed on Unit II system piping, its inclusion in Unit I technical specifications was not warranted.

Consequently, its removal while Unit II was in cold shutdown (Mode 5) where operability was not required was permitted.

Therefore, the public health and safety were not affected.

Cause of Event At the time of removal the snubber in question was listed in both Unit I and Unit II technical specifications although it was installed on Unit II piping of a common system.

The personnel involved were not aware that this snubber was listed in both units technical specifications.

Page 1 of 2 The control room bottled air system has four banks of air bottles with 42 bottles in each bank.

When either Unit 1 or Unit 2 is in opera-tional Mode 1, 2, 3, or 4, Technical Specification 3.7.7.1 requires at least 84 bottles pressurized to at least 2300 psig to be aligned to the control room.bottled air discharge header.

On May 14, 1981 an inspection of the bottled air system showed that only 83 bottles were properly aligned.

One bottle in one of the two banks aligned to the control room discharge header had been isolated.

Unit 1 was operating at 100 percent power and Unit 2 was in the cold shutdown condition when the event.was discovered.

This event is reportable pursuant to T.S. 6.9.1.9.b.

A subsequent investigation of the event conducted on June 9, 1981 revealed that a periodic test (2-PT-7'6.3) had been--performed twice on the Unit 2 portion of the bottled air system after, the failure of at least one bottle. The periodic test require<L:iierification that 42 are aligned to each bank manifold and verification that the manifold pressure for each bank is at least 23DO psig.

The accepta'nce criteria of the periodic test requires each bank to contain "... a minimum of 42 bottles of air each pressurized to at least 2300 psig." Both tests indicated satisfactory test results despite comments on one test critique sheet indicating that two bottles were isolated from their respective bank manifolds.

This event is reportable pursuant to T.S. 6.9.1.9.c.

Probable Cbnseouences of Occurrence 1

The reduction in system capacity from the loss of one bottle is negligible.

Cause of Event The high containment air temperature on June 1 was caused by a temporary loss of chilled water flow to the containment recirculation air cooling coils while changing valve line-up from the mechanical chiller to the steam chiller. The high temperature on June 3 resulted when the steam chiller was iecured and the mechanical chiller was placed in service on the Unit 1 containment in parallel with Unit 2.

Immediate Corrective Action In each case, the containment air temperature was reduced by valving in the desired chiller unit to provide the required chilled water flow.

Scheduled Corrective Action A higher heat load appears to exist on Unit 1 containment.

Corrective action will be to inspect insulation on various components such as the reactor head and steam generator supports during the next refueling.

. At 1452 on June 6, 1980 with the unit at Hot Standby, the "2..J

emergency diesel generator was removed from service for replacement of a transformer which supplies control power to the auxiliary lube oil pump in accordance with the ACTION statement of T.S. 3.8.1.1, Probable Consequences of Occurrence Since operability of the "2R" emergency diesel generator and both offsite power sources were verified within one hour of the removal of "2J" diesel from service, the health and safety of the public were not affected.

Cause of Event The "2.J"' emergency diesel generator was removed from service to replace a transformer which had failed.

The cause of the failure of the transformer could not be determined, however it was discovered that the 120/240 VAC transformer had a rating of 0.075 "K!vA and that there were 15 amp fuses in the power supply line to the transformer.

This size fuse was supplied by the manufacturer but no specifications for fuse size could be found.

The manufacturer was notified and is investigating why such a large fuse was supplied.

Immediate Corrective Action Both offsite power sources were verified and the "2H" emergency diesel generator was verified to be operable with.in 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> prior to the removal of the "2..J diesel from service and the transformer was replaced.

The relay which the transformer powered was also replaced due to damage it received when the transformer failed.

The 2.J" eme:-gency diesel generator was tested and returned to service 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and 16 minutes after being removed from service.

Scheduled Corrective Action None scheduled at this time pending manufacturer's review of fuse sizing.
