50-281 REPORT NO:

81-026/031~0 EVENT DATE:

04-29-81 TITLE OF EVENT:

CHARGING PUMP SERVICE WATER PUMPS INOPERABLE

1.

DESCRIPTION OF EVENT

On April 29,1981, with the unit at 100% power, charging pump service water pump, 2-SW-P-lOB, was removed from service in accordance with Design Change 80-42, Service Water to Control Room and Relay Room Air Conditioning and Charging Pump Coolant Piping Modification.

This* was* done so that a new bronze check valve could be installed.

WFi.en the valve was removed, water splashed down on the redundant pump, 2-SW-P-lOA, causing an electrical short and thereby making 2-SW-P-lOA inoperable.

With both Charging Pump Service Water Pumps inoperable, a Rampdown was immediately initiated.

This event is contrary to T.S. 3.3.A.8.b and is reportable in accordance with T.S. 6.6.2.b(2).

2.

PROBABLE CONSEQUENCES and STATUS of REDUNDANT EQUIPMENT:

The service water pumps, 2-SW~P-lOA & B, provide cooling water to the charging pump intermediate seal coolers and lubricating oil coolers.

During the period when both pumps'were-inoperable, charging pump temperatures were closely monitored.

There were** no observable increases in charging pump temperatures.

Therefore, the health and safety of the public were not affected.

3.

CAUSE

Water entering the top of 2-SW-P-lOA, hegause of a missing drip shield, created.an electrical short in. the.motor windings.

4.

IMMEDIATE CORRECTIVE ACTION

The immediate action was to replace the removed check valve and return the redundant pump, 2-SW-P-lOB, to service.

The unit rampdown was terminated 19 minutes after initiation.

5.

SUBSEQUENT CORRECTIVE ACTION:

An operable Service Water Pump was removed from Unit one and installed in place of 2-SW-P-lOA.

6.

ACTION TAKEN TO PREVENT RECURRANCE:

- The missing drip shield has been replaced.

7, GENERIC IMPLICATIONS:

None.
