INOPERABLE CONTAINMENT VACUUM PUMPS
- 1.
DESCRIPTION OF EVENT
e On December 12, 1980, the annunciator alarm "Cont Partial Press -0.1 PSI Chl" was received.
This was assumed to disable only the automatic containment Va-cuum Pump functions therefore no urgent actions were taken.
At 0815, it was discovered*that neither pump could be operated and preparations were begun to ramp the unit down..
Simultaneously, a broken lug was discovered on Relay CV-101 A 8 and a jumper was installed to allow one of the pumps to operate.
The lug was subsequently replaced, the jumper removed and the circuit returned to normal.
Operation with Containment VAcuum Fumps inoperable is contrary to T. S. 3.15.B and is reportable as per T.S.6.6.2.b.(2).
- 2.
PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT EQUIPMENT:
The relay, CV-101 A 8, when de-energized prevents operation of either Vacuum pump.
The annunciator for Containment Pressure "e.l PSI greater than Setpoint" was operational throughout the event, therefore the operator *would have been.
alerted to take action on any increasing Containment Pressure.
The health and safety of the public were not affected.
- 3.
CAUSE
A broken lug was found on the Relay Coil Neutral Lead.
- 4.
IMMEDIATE CORRECTIVE ACTION
A jumper was installed to bypass the failed Relay and allow the pumps to operate.
- 5.
SUBSEQUENT CORRECTIVE ACTION:
The broken lug was replaced, and the jumper removed.
- 6.
ACTION TAKEN TO PREVENT RECURRANCE:
A design change has been initiated to allow manual operation of the vacuum pump with a failure of the control circuit.
