SUBJECT:

MOV-CW-200A INOPERABLE

1. DESCRIPTION OF EVENT:

On 22 November, 1980, with Unit No. 2 at 100% power the outlet butterfly valve for "A" water box failed to operate. The valve had been po-sitioned to appro.xi-mately 75% open. Subsequently it failed to respond to both open and close demands. This is reportable Per Tech. Spec. 6.6.2.b.(2).

2. PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT EQUIPMENT:

The condenser water box isolation valves close during a LOCA in coincidence with a loss of offsite power to preserve the water in the intake canal for ECCS com-ponents. The inlet butterfly* valve (MOV-CW-206A) was operable and would have closed. Therefore, the health and safety of the public were not affected.

3. CAUSE:

The MOV's torque switch was found to be corroded and stuck in the open position.

4. IMMEDIATE CORRECTIVE ACTION:

An operator in radio contact with the control room was stationed at the valve to immediately close the valve if required.

5. SUBSEQUENT CORRECTIVE ACTION:

The torque switch was replaced and the valve tested operable.

6. ACTION TAKEN TO PREVENT RECURRENCE:

None required.

7. GENERIC IMPLICATIONS:

None
