50-281 REPORT NO:

80-007/03L-0 EVENT DATE: 06-06-80 TITLE OF REPORT:

SMOKE DETECTORS INOPERABLE

1.

Event Description

With Unit 2 at cold shutdown, all the Unit 2 smoke detectors were discovered to be deenergized.

This is contrary to T.S.3.21.A.l and is reportable per T.S. 6.6.2.b(2).

2.

Probable Consequences:

Upon discovery of the deenergized breakers the affected areas were immediately walked down and every hour thereafter until power was restored to the detectors.

There were no fire hazzards found, therefore the health and safety of the general pub lie were not affected.

3.

Cause

When the smoke detectors were found deengergized their supply of power was traced to distribution panel 2 MR-1 which was also found deenergized.

This panel is fed from the 2Cl-2 480V supply breaker on the 2Cl distribution switch board.

This breaker was found open.

Although it is possible for someone to have accidentally opened this breaker, it is unknown how it was opened.

The breaker was reclosed and no further problems have been encountered.

4.

Immediate Corrective Action

The immediate corrective action was to walk down the affected areas.

5.

Subsequent Corrective Action:

The subsequent corrective action was to investigate the source ot electrical power to the smoke detectors to find the cause of the loss of power and correct it.

6.

Future Corrective Action:

Because the breaker has not experienced any additional problems, no future corrective action is deemed necessary at this time.

7.

Generic Implications:

None.
