Serial No. 7 49 PO/FHT:scj Docket No. 50-2~0 License No. DPR-32 We have reviewed your letter of September 7, 1979, in reference to the inspec-tion conducted at Surry Power Station Units 1 and 2 on July 30 - August 3, 1979, and reported in IE Inspection Report Nos. 50-280/79-45 and 50-281/79-64.

Our responses to the specific infractions are attached.

We have determined that no proprietary information is contained in the report.

Accordingly, the Virginia Electric and Power Company has no objection to these inspection reports being made a matter of public disclosure.

NRC COMMENT 9 :*C2 As required by Technical Specification 6.4.A and Virginia Electric and Power Company (VEPCO) QA Manual Section 5, detailed written procedures with appropriate check-off lists and instructions shall be provided for preventative or corrective maintenance operations which would have an effect on the safety of the reactor.

Section 16 of VEPCO QA Manual de-fines maintenance as including those activities performed by maintenance

. or contractor personnel to maintain or repair safety~related equipment.

Contrary to the above:

1.

General Welding Procedure Number W-1.0, which requires detailed proce-_

dures for major repairs, does not provide a definition for minor or major repairs.

2.

Weld repair corrective maintenance operations were being conducted without detailed mechanical maintenance procedures required by QA Manual, Section 16.

3.

The mechanical maintenance procedure for the dismantling of valves in

  • the steam generator feedwater lines did not contain sufficient detail t<;> prevent damage to the valve body during removal of a welded seal membrane.

This is an infraction.

RESPONSE

The above infraction is correct as stated.

1.

Corrective steps which have been taken and the results achieved:

a.

The General Welding Procedure, W-1.0, has been modified to inclu~e the definition of a minor and major weld repair.

b.

A new procedure has been generated, MMP-C-G083, to cover minor weld repairs.

c.

Mechanical Maintenance Supervisors have been re-instructed on the correct method of using the General Valve Maintenance Procedure, MMP-C-G-001.

This procedure require~ the supervisor to provide the "detailed" instructions required to effect.the repair in step 5.4.

2.

Corrective steps will be taken to avoid further non-compliance:

The actions taken above will preclude further non-compliance.

3.

The date when full compliance will he achieved:

Full compliance has been achieved.

Contrary to the above:

1.

As of July 31, 1979, the liquid penetrant and visual inspections of repair excavations required prior to welding by General Welding Pr_ocedure Number W-1. 0 were not being done.

2.

The cable trays and ventilation ducts in the area of the feed water line valves were damaged when used as scaffolding and work platforms, despite Maintenance Procedure MMP-C-G-001 requirement that surrounding -

equipment be adequately protected.

This is an infraction.

RESPONSE

The above infraction is correct as stated.

Corrective steps which have been taken and results achieved:

Welding supervisors have been re-instructed on the required NDT examinations of excavated repair areas.

b. *All supervision*associated with maintenance related activities have been instructed on the importance of utilizing proper scaffolding and protecting plant systems and components.

Corrective steps which will be taken to avoid further non-compliance:

The actions taken above will preclude further non-compliance.

3.

The date when full compliance will be achieved:

Full compliance has been achieved.
