The review should consider whether the report identifies a generic defect or problem with the package design and the safety significance of the issue.
Note that a high safety significance represents a potential for significant radiation exposure, medium safety significance represents a potential for some moderate radiation exposure, and low safety significance represents little or no potential for radiation exposure.
- 1. The report identifies:

- 3. Summary of the report:
From March 2012 through September 2017, URENCO USA (UUSA) made shipments of 30B transport cylinders (Type B) with cylinder valve gaskets to multiple customers that did not meet American National Standards Institute (ANSI) N14.1-2012, Packaging of Uranium Hexafluoride for Transport cap gasket specifications. UUSA found the non-conforming cap gaskets to have a larger thickness and smaller diameter than the gasket specified in ANSI N14.1-2012. The purpose of the cap gasket is to prevent any loss or dispersal of possible contamination from the valve's internal space and also to protect the valve from intrusion of dirt and/or water from the outside. The cap gasket is not part of the pressure retaining containment of the valve.
URENCO, UUSA's parent company, operates uranium enrichment facilities worldwide.
UUSA's plant design was based on the URENCO Netherlands (UNL) plant. In 1980, UNL created a deviating drawing which served as the basis for the supply of gaskets to the facility.
The modified drawing was supplied to UUSA as part of the hand over from UNL to UUSA prior to commencing commercial operations. Following review of the drawing, against standards prior to procurement for use, UUSA adopted the UNL drawing as a design basis.
The cause for the condition prompting this report was that the UUSA drawing review was not performed adequately to discover the discrepancy.
Conformance to this standard is required by Condition 6 (requiring conformance to ANSI N14.1-2012) and Condition 9 (gasket inspection) of the Certificate of Compliance No. 9196 (pertaining to docket number 71-9196) for the UX-30 transportation package which is used to transport the cylinders.

Following the discovery, UUSA notified the affected fuel fabrication facilities of the issue and of associated corrective actions on January 11, 2018. The affected customers UUSA informed include: Global Nuclear Fuel - Americas, LLC, Westinghouse Electric Company, LLC, and Framatome, Inc..
Each of the affected customers also submitted reports pursuant to Title 10 of the Code of Federal Regulations (10 CFR) 71.95 reports for return shipments made of heels with the nonconforming gaskets.
- 4. Corrective actions taken by the licensee:
The deviations were self-identified by UUSA on September 12, 2017, at which point conformance was reestablished. UUSA documented a report of the nonconformance and notified the affected fuel fabrication facilities. The report includes the following corrective actions:
- Replace incorrect internal drawing with ANSI N 14.1 compliant drawing to ensure future purchases for the gasket are in accordance with ANSI 14.1-2012. This action has been completed.
- Stop shipments of cylinders with noncompliant gaskets. This action has been completed.
- Procure ANSI N 14.1 compliant gaskets and purge all noncompliant gaskets from plant. This action has been completed.
- 5. Staff comments:
As there were no incidents during the referenced shipments, the safety significance of this nonconformance is considered low. The package integrity was not substantially compromised, and there was no exposure of individuals to radiation.
- 6. Staff conclusion:
The report does NOT identify generic design or license/certificate issues that warrant additional Commission or staff action. This report is considered closed.
