As required by Amendment No. 46 to Unit 2's operating license issued January 24, 1979, the Health Physics Program and procedures which have been established .for the Steam Generator Replacement Program shall be implemented. The Health Physics Manual for the Steam Generator Replac~

ment Project, Procedure* HP-SGRP-10; *Paragraph B.2*.d, :requires that * * **

test and checks of tents be performed by Health Physics and an acceptance or "stop" sticker affixed to the tent.

  • Contrary to the.above, the tents located in the ~18 foot elevation of Unit 2 being used for the removal of slag from the reactor coolant

. system pipe prior to decontamination of. the pipe and .being used. for working miscellaneous piping had not been inspected nor an acceptance label affixed on April 11, 1979 *

Tents were inspected, found satisfactory and an acceptance label was affixed on April 11, 1979.

2. Corrective steps taken to avoid further violations:

A copy of Procedure HP-SGRP-10 was placed in.the Health Physics required reading file.

3. Date when full compliance will be achieved:

Full compliance has been achieved

As required by 10 CFR 20.20l(b), each licensee shall make or cause to be made such surveys as may be necessary for him to comply with the regulations in this part.

Contrary to the above, radioactive liquid effluents from the plant's subsurface drain system were automatically pumped to the discharge*

canal between January 1, 1979 and April 11, 1979, without performing radioactivity analys~s of the effluents at a frequency adequate to ensure that 10 CFR 20 limits are not exceeded.

"This is an infraction and applies to Units 1 and 2.

B. Response:

The item is correct as stated.

Subsurface drain system sampling frequency was changed to daily on April 19, 1979.

The Periodic Test procedure was revised to reflect a daily sampling requirement versus weekly.

3. Date when full compliance will be achieved:

Full compliance has been achieved.

C. NRC Comment:

As required by 10 CFR 20.203(b), each radiation area should be conspicuously.posted with a sign or signs bearing the caution symbol and the words "Caution - Radiation Area." 10 CFR 20. 202 (b) (2) states the "Radiation Area means any area,* accessible to

I Page 3 of 3 to personnel, in which there exists radiation, *** at such levels that a major portion of the body could receive in any one hour a dose in excess of 5 millirem **** "

Contrary to the above, on April 12, 1979, a radiation area near a piece of -reactpr coolant' system pipe located outside the Unit 2 equipment hatch was not posted as a radiation area. Radiation levels in accessible areas near the pipe were 10 - 20 mR/hr.

This is an infraction and applies to Units 1 and 2.
