1.

Description of Event

During a nonnal new fuel inspection, some discoloration and a white crystaline substance, later identified as a caustic solution, were found on sixty-two (62) of sixty-four (64) new fuel assemblies.

The fuel is for use in Unit 2 (Cycle SJ, which is presently shutdown and defueled.

This report is made in accordance with 10 CFR 73 Section 73.71 Paragraph (b)~

2.

Probable Consequences:

The**extent of damage, if any, to the new fuel:assemblies iscbeing*evaluated.

No. activity was released.*.

3.

Cause

The event was caused by the unauthorized introduction of a caustic solution in the new fuel storage area.

Investigation is still in progress.

4.

Immediate Corrective Action

All appropriate personnel*and investigating authorities were notified.

Samples of the foreign substance were taken and are being analyzed.

  • The new fuel has.been removed from the storage area and rinsed.

The assemblies.*are _being inspected for damage.

5.

Subsequent Corrective Action:

Access to the new fuel storage area has been limited.

Continued investigation will determine additional corrective action.

6.

Action Taken to Prevent Recurrence:

The investigation will determine what actions must be taken to prevent recurrence.

7.
