Station 50-281 79-009/03L-0 2/8/79 Snubbers Not Inspected 1.) Description of Event:

On 2/8/79, while performing PT-39B-l (Accessible Snubbers - Visual Inspection-As Found), the mechanic performing the inspection discovered three snubbers that were not listed on the PT.

Further investigation revealed that the three snubbers had not been visually inspected during the previous performance of either PT-39B-l or PT-39A-l (Accessible Snubbers Visual Inspection -

As Left).

This is contrary to Tech. Spec.

4.1.7.A and reportable in accordance with Tech. Spec. 6.6.2.b.(3).

The subject snubbers are 2-SI-HSS-21, 2-SI-HSS-24, and 2-SI-HSS-27.

2.)

Probable Consequences/Status of Redundant Systems:

Upon discovery, the three snubbers were inspected and determined operable. The health and safety of the general public were not affected.

3.)

Cause of Event

The three snubber~were inadvertantly omitted from both PT-39B-l and PT-39A-l when the PT's were changed on August 18, 1978.

4.)

Immediate Corrective Action

The three snubbers were added to PT-39B-l and inspected.

The snubbers were determined operable in accordance with the visual inspec-tion criteria stipulated in the PT.

5.)

Scheduled Corrective Action:

A request to change procedure has been initiated to permanently add the subject snubbers to both PT-39B-l and PT-39A-l.

6.) Action Taken to Prevent Recurrence:

A thorough review of all snubber PT's for both Unit 1 and Unit 2 has been performed.

The subject three snubbers were the only ones that were omitted.

Since this is considered a random event, there are no generic implications.
