1.

Description of Event

e With the unit at rated power, the "after opening" leak testing of personnel hatch inner door and emergency escape manway were not -tested within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> after opening.

After use, the inner door was closed, but the s9-fety lock on the escape manway was.believed, broken. Access to the personnel airlock was precluded for personn~l safety reasons, the inner door leak test can not be done unl~ss personnel are inside the air 1oc.k.

The outer door was tested satisfactorily. With the outer door sealed, it was tagged out to insure containment integrity.

Probable Consequences:

Since the outer door was tested satisfactorily and tagged out, containment integrity was not violated.

The health and safety of the general public were not affected.

3.

Cause

The test of the inner door and escape mammy was not performed because the ent-i;ance into the air lock posed a.*potential personnel hazard.

4.

Immediate Corrective Action

Containment integrity was verified by insuring*the outer door was properly sealed.

5.

Subsequent Corrective Action:

Tµe outer door was tagged out of service to ensure containment integrity and will remain tagged out until repairs to the inner door are completed during the next outage.

6.

Actions Taken to Prevent Recurrence:

None
