1. Description of Event

During a cold shutdown period, the valve pit, containing HOV-SW-202B, was flooded while draining the circulating water box D.

The valve operator was 3~~rted by the flooding and the valve became inoperable in its automatic_-

i:!Ode.

The valve is required to shut automatically on "LOCA-plus-blackout"

£or c~rculating ~a~er conservation.

-:::::,::':! -,--=2.,-=: ::C!DV-3W-202B incorporates an auto-close function as a means of conse=-:-'~g c::..::-=-~a=i__~g water during LOCA-plus-blackout ~onditions.

The valve was --....: ____ ::::e =2.uure was discovered and remained shut until the operator was -.-"'-:;--- -:-=-,-.

-:::.2 health and safety of the public were not affected.

Cause

--"' s 2-.-e::1:::: occurred because a drain line _could not accept the volume of water-::~~ i:ron condenser 2-CN-SC-lB (D-Waterbox) to station drain to facili=.s_te 2 -,.::-;ntenance operatio_n.

The water filled the valve pit, containing the 1*1017-s:;-2023, thereby shorting out the motor.

Immediace Cor=ective Action:

The valve pit was pumped out, and the motor tested for grounds caused by the water.

5.

Subsequent Corrective Action:

The motor was replaced and the valve was made operable in all respects.

6.

Actions Taken to Prevent Recurrence:

Personnel are instructed to control the pump discharge so as not to endanger adjacent equipment.

7.

Generic Implications:

None, the failure occurred due to water penetration of the electrical components.
