NRC INTEGRATED INSPECTION REPORT 50-272/99-04, 50-311/99-04

Dear Mr. Keiser:

On May 29, 1999, the NRC completed an inspection at your Salem 1 & 2 reactor facilities. The

enclosed report presents the results of that inspection. The preliminary results and conclusions

were presented to PSE&G management led by Mr. Lou Storz of your staff in an exit meeting on

June 4, 1999.

During the six-week period covered by this inspection report, your activities at Salem were

generally characterized by safety-conscious operations, sound engineering and maintenance

practices, and careful radiological work controls .

Overall, Unit 2 outage activities were effectively coordinated and performed safely with

appropriate radiological controls. Also, operators responded effectively to a Unit 1 automatic reactor trip that resulted from a dropped control rod. Nonetheless, we concluded that continued

management attention was warranted in human performance, as evidenced by the fact that two

issues in this area resulted in violations of NRC requirements. First, the inappropriate marking

of procedure steps as "not applicable* resulted in inadequate maintenance on the Unit 2

containment fan cooler units (CFCUs). Secondly, ineffective supervision of a surveillance test

resulted in an unplanned entry into technical specification 3.0.3. These events had negligible

actual safety consequence, but both could have been prevented by more alert supervision and

better questioning attitude by your staff.

We have determined that two Severity Level IV violations of NRC requirements occurred, as

discussed above. These violations are being treated as non-cited violations (NCVs), consistent

with Appendix C of the Enforcement Policy. These NCVs are described in the subject

inspection report. If you contest the violation or severity level of these NCVs, you should

provide a response within 30 days of the date of this inspection report, with the basis for your

denial, to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC

20555-0001, with copies to the Regional Administrator, Region I, and the Director, Office of

Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001; and

the NRC Resident Inspector at the Salem facility .
