NRC OPERATIONAL SAFETY TEAM INSPECTION REPORT 50-272/99-03,

50-311199-03

Dear Mr. Keiser:

The NRC conducted an Operational Safety Team Inspection (OSTI) at the Salem facilities from

March 15-19, 1999. The enclosed report presents the results of this inspection. On March 19,

1999 preliminary results were discussed with Mr. E. C. Simpson and other members of your

staff during an exit meeting held'at the Salem facility. The team performed a risk-informed,

performance-based inspection tcS evaluate specific activities in the Operations and Maintenance

functional areas, including corrective actions .

Overall the team concluded that operational and maintenance activities were acceptable to .

  • support continued safe plant operations. Operational activities were professionally conducted

and operations personnel continued to operate the plant safely during normal operations and

plant transients. Maintenance activities were properly conducted and provided adequate daily

support of operational activities. The team found that your oversight processes had recognized

a decline in performance associated with human performance errors, corrective action backlogs,

and work controls. Station management has been. responsive to these problems and has

implemented a number of recent initiatives to address these areas.

We found your corrective actions reasonable; however, additional time is necessary to validate

the effectiveness of your implemented initiatives. The team determined that corrective action

activities associated with significance level 1 corrective actions are not being adequately

resolved within established procedural goals. In order to ensure that corrective action activities

are accomplished in a timely manner continued management attention appears warranted. We

plan to assess your progress during future inspections.
