NRC INTEGRATED INSPECTION REPORT 50-272/98-06, 50-311 /98-06

Dear Mr. Keiser:

On August 1, 1998, the NRC completed an inspection at your Salem 1 & 2 reactor

facilities. The enclosed report presents the results of that inspection.

During the six-week period covered by this inspection, your conduct of activities at the

Salem facilities was generally characterized by safety-conscious operations, sound

engineering and maintenance practices, and careful radiological work controls. The

inspectors conducted a focused review of your radioactive waste management program,

including sampling, processing, classification, and shipment documentation, and found that

it was effectively implemented. Additionally, we viewed your decision to remove Unit 2

from service to install new design safety valves as appropriate and conservative. The

inspectors noted that operators performed well during the plant shutdown and cooldown

activities.

We are concerned, however about an apparent violatio.n of maintenance procedure

requirements that led to a failure to meet the technical specification operability

requirements for the auxiliary feed water (AFW) system. In this case, maintenance

technicians failed to restore an isolation valve for an AFW pump discharge pressure

transmitter to the open position following a calibration activity. This issue is viewed as

significant since an independent verification o.f the valve's position by a second technician

failed to identify the error, and because the improper valve position was discovered only

when operators attempted to place the AFW system in service to support the Unit 2

shutdown. Additionally, this concern appears to be repetitive in that similar examples of

inadequate control of instrument valves and other plant equipment have been identified

within the last year at both your Salem and Hope Creek facilities. We intend to review

yOur forthcoming Licensee Event Report describing this issue for corrective actions prior to

making a final enforcement decision in this case.
