NRC INTEGRATED INSPECTION REPORT 50-272/97-15, 50-311/97-15
Dear Mr. Eliason:
On September 7, 1997, the NRC completed an inspection at your Salem 1 & 2 reactor
facilities. The enclosed report presents the results of that inspection.
During the 6-week period covered by this inspection report, your conduct of activities at
the Salem facilities was generally characterized by safety-conscious operations, sound
engineering and maintenance practices, and careful radiological work controls. On August
6, * 1997, you gained NRC approval for the restart of Salem Unit 2 and Confirmatory Action
Letter 1-95-009 was modified. Unit 2 achieved criticality on August 17. Overall,
personnel and plant management controlled the reactor start-up evolutions well. The
operating crews were attentive, used excellent communication skills and appropriately
addressed all issues and challenges. In addition, management provided strong oversight
and self assessment of plant activities. However, maintenance planning weaknesses were
identified during the troubleshooting of a pressurizer level transmitter. We were particularly
concerned about the decision to enter the Unit 2 biological shield to isolate the .level
transmitter when acceptable alternatives were available that would have significantly
reduced the workers' exposure. We understand that you acknowledge that the decision
was not appropriate and have initiated an action request to review the troubleshooting
plan.
