NRC INTEGRATED INSPECTION REPORT 50-272/96-18, 50-311/96-18

NOTICE OF VIOLATION *

Dear Mr. Eliason:

    • On January 25, 1 ~97, the NRC completed .an inspection at your Salem 1 & 2 reactor

facilities. The enclosed report presents the results of that inspection.

During* the 6-week period covered by this inspection period, your conduct of activities at

the Salem facilities was generally chara:lcterized by safety-conscious operations, sound

engineering and maintenance practices, and careful radiological work controls .. We noted,

in particular, the generally good operator performance during four major evolutions as

operators began to transition the plant from outage activities to power operation .

. Significant improvement was noted in the performance of the Quality Assurance

Department. We view this improvement as especially significant when compared to the

ineffectiveness of that organization prior to rriid-1995. *Your efforts to improve the

Emergency Preparedness Program were found to* be suffici~nt to support Salem restart.

We are concerned, however, about three violations Qf NRC requirements tnat we

identified. We are particulariy concerned ab~ut the*loss of containment closure during

Salem Unit 2 refueling stemming.from i!'leffective work control. Although no safety

consequence resulted fro*m the loss-of containment c'losure, the problem accentuates the

potential impact of ineffective *work control on safe plant operation. Since you have not

yet completed implementation of y~ur planned corrective action for past work control

deficiencies, and in light of the problem documented in this report, we remain concerned

about your ability to control work to insure safe plant operation ..

Although we noted several examples of improved engineering performance, we also noted

two violations involving the Nuclear Engineering Design Department. Although we

consider engineering good overall, we conclude that your efforts to continua improvement

important for long term engineering. success.

These violations are cited in the enclosed Notice of Violation, and the circumstances

surrounding the violations are describt.d in detail in the* enclosed report. Please note that

you are*required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part,

  • to determine whether further enforcement action is necess~ry to ensure compliance with

regulatory requirements.
