SALEM LICENSING BASES TEAM INSPECTION REPORT 96-80
Dear Mr. Eliason:
This letter transmits the report of the U.S. Nuclear Regulatory Commission (NRC) Special
Team Inspection (STI) at Salem Unit 2, and various follow-on inspection activities
conducted in several phases between May 6 and October 25, 1996.
The preliminary findings of the May 1996 team were initially discussed at a meeting
between Mr. B. Simpson and other members of your staff, and Mr. J. Wiggins and other
NRC representatives, on May 23, 1996. The May 1996 team found examples in the
Unit 2 control air, fuel handling ventilation, and containment cooling systems where the
design and licensing bases were not well understood and connected. A meeting was held
on June 11, 1996, to discuss the preliminary findings of this team and, more broadly, your
actions with respect to the conformance of the Salem Station to its licensing basis. The
extent and significance of these issues were such that you supplemented your current
outage efforts by initiating an "FSAR Project" to examine licensing bases conformance in a
systematic manner. Aspects of that project, including 17 specific NRC questions, were
discussed in telephone conferences and are summarized in your August 23, 1996 letter to
the NRC. We understand that the information from your FSAR project, coupled with the
results of previously completed as-built walkdowns, provided the basis for your conclusion
that there is reasonable assurance that Salem Unit 2 could be operated within its
licensing/design bases. That conclusion was subsequently presented to your Management
Review Committee (MRC) and approved on September 30, 1996. NRC management will
review the results of both the SSFI ano the inspection reported herein, in reaching its
conclusion in these matters.
The NRC observed portions of the FSAR Project in progress (from July-September 1996)
and reviewed a sample of its outputs. During the week of October 21, 1996, progress in
addressing specific items from this team was also examined by the NRC. Our observations
of the FSAR Project found it to be a substantial effort that was well managed. The Project
included well-focused elements of independent confirmation. Nevertheless, to provide an
independent review of your efforts and a measure of additional assurance that Salem
Unit 2 can be operated within its licensing/design bases, the NRC initiated a Safety System
Functional Inspection (SSFI), which will be reported upon separate from this effort.

Several findings from this inspection represent violations of NRC requirements, but are not
being cited in coordination with the Office of Enforcement, in accordance with the criteria
of Section Vll.B.2 of the NRC Enforcement Policy, principally because they stem from
licensed activities prior to the shutdown period and have since been addressed by broad
comprehensive PSE&G programs underway at Salem to identify and correct such problems.
In accordance with 10 CFR 2. 790 of the Commission's regulations, a copy of this letter
and its enclosure will be placed in the NRC Public Document Room.
