NRC INTEGRATED IN')PECTION REPORT 50-272/96-07, 50-311/96-07

NOTICE OF VIOLATION

Dear Mr. Eliason:

On June 29, 1996, the NRC completed an inspection at your Salem 1 & 2 reactor

facilities. The enclosed report presents the results of that inspection.

During the 6-week period covered by this inspection period, your conduct of

activities at the Salem facilities was generally *characterized by safety-

conscious operations, sound engineering and maintenance practices, and careful

radiological work controls. However, operators did not demonstrate a thorough

questioning attitude when evaluating three plant activities.

We are also

concerned about two violations of NRC requirements pertaining to failure to

comply with procedure requirements and failure to make a required report for a

defective plant component.

We also noted a number of examples of poor quality maintenance during the

inspection period. This -has bee*n a recurring problem in recent Salem history.

We are especially concerned, since equipment performance problems precipitated

and complicated many of the transients that have occurred at Salem.

The

problems in maintenance resulted, in part, from the procedure non-compliances

noted in the attached Notice of Violation.

We noted that you have initiated

comprehensive measures to correct the quality of maintenaDce.

During a followup inspection of a Magne-Blast circuit breaker performance

problem, the NRC determined that your staff did not make a required report per

10 CFR 21.

This problem indicated less than effective communication between

the engineering and licensing organizations, as well as *less than adequate

procedures regarding the reporting requirements of 10 CFR 21. This

constituted.a violation of 10 CFR 21 reporting requirements.

You are required

to respond to this matter in accordance with the enclosed Notice of Violation.
