NRC INSPECTION NOS. 50-311/96-01

Dear Mr. Storz:

This letter refers to your April 25, 1996 correspondence, in response to our March 25, 1996 letter.

Violation A listed four examples of failure to follow procedures.

In your response, you listed causes and corrective actions specific to each of the examples (2 personnel errors, 1 inattention to detail, and 1 inadequate procedure).

Your actions to prevent recurrence did not address the broader, generic issue of inadequate procedure use and inadequate procedures. Thus, within 30 days of your receipt of this letter, we ask that you reconsider this matter and either amend your response or reaffirm your existing response to specifically include your basis for asserting that weaknesses in this area were not generic.

Your response to Violation B was acceptable and will be examined during a future inspection of your licensed program.

Violation C listed two examples of inadequate cor~ective action. Again, the response was specific to the individual issues.

Example 2 specifically stated that corrective action from four previous service water pipe fires was inadequate.

Your response indicated which of your previous corrective actions were inadequate, but not the cause of the inadequate corrective action (such as inadequate commitment tracking,

  • lack of management follow-through to insure corrective actions are implemented, etc.). Thus, your actions to prevent recurrence did not address the broader, generic issue of why your corrective actions were inadequate.

Therefore, within 30 days of your receipt of this letter, we ask that you reconsider this matter and either amend your response or reaffirm your existing response to specifically include your basis for asserting that your original response was adequate.
