SALEM INSPECTION NOS. 50-272/95-81; 50-311/95-81 AND

NOTICE OF VIOLATION

Dear Mr. Eliason:

The enclosed report documents a special inspection for public health and

safety, conducted by John A. Calvert, Lonny L. Eckert, and Jenifer M. Shannon

at the Salem Nuclear Generating Station for the period between

October 5, 1995, and November 13, 1995.

The inspectors discussed the findings

of this inspection with Messrs. L. Storz, Senior Vice President-Nuclear

Operations, C. Warren, General Manager-Salem Operations, and other members of

your staff.

The inspection covered the emergency preparedness, root cause, and significant

event response team activities concerned with the loss of the overhead

annunciator system event for Salem Unit 1. Within these areas, the inspection

consisted of interviews with personnel, observation of activities in progress,

and examination of procedures and representative records.

We found several weaknesses in your emergency preparedness program.

The

inspectors noted a failure to promptly declare an alert, a failure to fill all

key emergency response organization positions in a timely manner, and a

failure to discuss revisions of the EAL scheme with the States and acquire

their agreement to the changes.

The inspectors also concluded that your

emergency response organization call-out process was weak as shown by, for

example, the fact that a duty call list was not maintained, and a significant

number of those people that were called failed to respond or questioned the

need to respond.

In this event, the inspectors concluded that there was no indication to

operators of major overhead annunciator system problems that would prevent

alarm processing.

The inspectors observed an overall weakness in the

effectiveness of the engineering, operations, and training organizations to

support the plant operators with a unified set of overhead annunciator system -

knowledge, skills, and abilities to recognize failure indications, to

determine operability, and to take proper corrective action .

We understand that the New Jersey Bureau of Nuclear Engineering (BNE) was not

satisfied with the quality of information which was provided by your staff

during the event.

We recognize that your staff has ret with New Jersey BNE

since the event.

We encourage you to work with them to address their

concerns.

Based on the results of the inspection, the NRC has determined that violations

of NRC requirements occurred.

The violations are cited in the enclosed Notice

of Violation (Notice) and the circumstances surrounding it is described in

detail in the subject inspection *report.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence.

Your response may reference or

include previous docketed correspondence, if the correspondence adequately

addresses the required response. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

At the November 13, 1995, exit meeting, your staff indicated to us that the

emergency preparedness related issues would be addressed prior to the restart

of your Salem units.

As such, we request that you provide to us in writing

your plans and schedules to address the emergency preparedness related issues

identified by your significant event response team (SERT) and by this report

within 30 days of receiving this letter. This request is in addition to the

required response to the Notice of Violation.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of

this letter, its enclosure, and your response will be placed in the NRC Public

Document Room (PDR).

To the extent possible, your response should not include

any personal privacy, proprietary, or safeguards information so that it can be

placed in the PDR without redaction.

The responses directed by this letter

and the enclosed Notice are not subject to the clearance procedures of the

Office of Management and Budget, as required by the Paperwork Reduction Act of

1980, Pub. L. No. 96.511 .
