SALEM RESIDENT INSPECTION NOS. 50-272/95-17; 50-311/95-17 AND
Dear Mr. Eliason:
The enclosed report documents an inspection for public health and safety,
conducted by Mr. C. Marschall, Senior Resident Inspector and other members of the
NRC resident and regional staff at the Salem Nuclear Generating Station for the
period between August 13, 1995 and October 14, 1995.
The inspectors discussed
the findings of this inspection with Messrs. C. Warren, General Manager-Salem
operations, and other members of your staff.
This report also contains inspection findings of Ms. C.D. Beardslee of the
regional inspection staff, as well.as findings of our system reviews which were
performed during the time period indicated.
The overall conclusions are
reflected in the Executive Summary for the complete report.
We are taking this
approach to better integrate our inspection findings into a consistent regulatory
message and more clearly communicate how these findings may reflect a change in
your performance.
Both units were operated safely during this period.
We were encouraged by the
actions of the operators to activities that did not meet expected performance
standards.
Additionally, we observed an improving trend regarding control of
outage risk. During the inspection we monitored your System Readiness Reviews
and the Management Review Committee's review of the items recommended for action
prior to restarting Salem Unit 1. We found that the System Readiness Reviews and
oversight by the Management Review Committee provided a sound method for
determining the focus of maintenance and modifications to improve plant safety.
Based on the results of an inspection of your tagging and work control, the NRC
has determined that a violation of NRC requirements occurred. The violation is
cited in the .enclosed Notice of Violation (Notice) and the circumstances
surrounding it is described in detail in the subject inspection report.
The
violation is of concern because your previous attempts to correct problems with
tagging and work control have not been effective.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. In your response,
you should document the specific actions taken and any additional actions you
pl an to prevent recurrence.
Your response may reference or include previous
docketed correspondence, if the correspondence adequately addresses the required
response. After reviewing your response to this Notice, including your

proposed corrective actions and the results of future inspections, the NRC will
determine whether further NRC enforcement action is necessary to. ensure
compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's
11Rules of Practice", a copy of this
letter, its enclosure, and your response will be placed in the NRC Public
Docu~ent Room (PDR).
To the extent possible, your response should not include
any personal privacy, proprietary, or safeguards information so that it can be
placed in the PDR without redaction. The responses directed by this letter and
the enclosed Notice are not subject to the clearance procedures of the Office of
Management and Budget as required by the* Paperwork Reduction Act of 1980, Pub.
