January 26, 1995 Here is the information you requested concerning our Bulletin 88-08 response.

A copy of the test used to verify flow meter indication is attached.

If indicated flow reaches 0.05 gpm, PSE&G will troubleshoot at that time.

Probable actions will be close SJ4 and SJ5 valves upstream of the SJ12 and SJ13 valves.

If the flow decreases, than leakage can be attributed to leakage through either SJ12 or SJ13.

An evaluation will be performed at that time to consider continuing operation*with SJ4 and SJ5 closed and SJ12 and SJ13 open.

All four valves are actuated by a Safety Injection and tested as required.

Therefore, there would be no decrease in safety or functionality.

If the leak does not stop when SJ4 and SJ5 are closed, the leakage is probably pressure isolation valve leakage from the Reactor Coolant System and Salem would be governed by the technical specifications associated with Reactor Coolant System leakage.

Flow meters are replaced every five years.

WORK STANDARD FOR FLOW METERS 2FI928A AND 2FI928B (11/16/94)

This work standard will be attached to the work order and be used to (1) verify that an unobstructed flow path exists from the safety injection lines to the 2FI928A and 2FI928B and (2) to verify the capability of the flow meter 2FI928B to detect low flow conditions.

This information is required to meet the Salem commitments to NRC Bulletin 88-08.

The activities described herein shall fall within the normal skill level of the personnel assigned to perform this task.

Precautions:

a.

The Holdup tank volume is sufficient to accommodate the pipe and potential leakage volume.

b.

The Safety Injection System is not required to be OPERABLE.

c.

The work standard requires a low flow pump.

The flows range from.5 gpm to.05 gpm.

d.

Take adequate precaution to ensure there is no cross contamination between safety injection system and demin water system.

Work Standard:

Valve line-up:

Close or verify closed the following valves:

Allow line to drain to Holdup Tank until zero flow is indicated on the 2FI928A and 2FI928B flow meters.

Open the 2SJ166 test line isolation valve.

Drain any residual fluid until the 2FI928A and 2FI928B flow meters read zero.

Connect the test pump to drain valve 2SJ310.

The source of water should be demin water.

Open the 2SJ309 and 2SJ310 drain valves.

Perform an integrity test to ensure proper isolation.

Verify pump flow independent of flow meters 2FI928A and 2FI928B at decreasing pump flows of.5 gpm,.4 gpm,.3 gpm,.2 gpm,.1 gpm and.05 gpm.

A calibrated instrument should be used.

Identify the instrument used.

Record instrument identification and pump flows on Attachment, Part A, Column A, Items 1 through

6.

For each pump flow rate in Step #7, read the indication on flow meter 2FI928B (0 to 1 gpm).

Record the 2FI928B flow rate reading for each *of these pump flows in Part A, Column B, Items 1 through 6 of the Attachment.

9.

Verify pump flow independent of flow meters 2FI928A and 2FI928B at increasing pump flows of.05 gpm,.1 gpm,.2 gpm,.3 gpm,.4 gpm, and.5 gpm.

Record pump flows on Attachment, Part B, Column A, Items 1 through 6.

10.

For each pump flow rate in Step #9, read the indication on flow meter 2FI928B (0 to 1 gpm).

Record the 2FI928B flow rate reading for each of these pump flows in Part B, Column B, Items 1 through 6 of the Attachment.

11.

The above readings should be read by the technician and independently verified.

Both individuals shall sign the Attachment as indicated.

12.

Return the valve line-up to the original configuration.

13.

Provide the completed Attachment to J. Weidemann, Salem Technical when completed.
