SUBJECT:

NRC INSPECTION 94-21 {REPLY)

This refers to your, January 16, 1995, correspondence in response to our

letter, dated November 30, 1994, regarding Salem Units 1 and 2.

This

correspondence dealt with your failure to perform inservice tests {IST) of

certain safety-related check valves in accordance with 10 CFR 50.55a{f).

Also, we found two instances where actions to prevent recurrence of conditions

adverse to quality were ineffective. These involved post-maintenance

verification of pump baseline data and premature lifting of certain safety-

related relief valves. These items were identified during a review of your

IST program conducted under NRC Temporary Instruction 2515/114, "Inspection

Requirements for Generic Letter 89-04, Acceptable Inservice Testing Program."

We have reviewed these matters in accordance with NRC Inspection Manual

Procedure 92903, "Engineering."

Your evaluation identified as root causes personnel error/inattention to

detail, ineffective administrative controls, and inadequate problem review and

followup.

Your actions to prevent recurrence included preparation of a relief

request pertaining to check valve testing, strengthened administrative

controls, personnel training, increased IST program staffing, and plans to

perform a self-assessment of the IST program.

We concur with your assessment of the root causes outlined above, and consider

the actions you have taken to be acceptable.

However, we note that your

current test schedule for check valves 1SJ3 and 2SJ3 will continue to conflict

with the requirements of 10 CFR 50.55{f) until the NRC has approved your

proprised relief request.

The success of your corrective actions, including the degree to which

management oversight of your IST program is effective in preventing future

violations, will be reviewed in future inspections.
