COMBINED INSPECTlON REPORT NOS. 50-272/95-01 AND 50-311/95-01

Dear Mr. Eliason:

This letter refers to the announced safety inspection conducted by

Mr. L. Scholl and others of this office from January 12-20, 1995, at Salem

Nuclear Generating Station, Units 1 and 2.

The inspectors discussed our

findings with members of your staff at the end of the inspection.

Areas examined during this inspection are described in the enclosed inspection

report. The inspection was a review and assessment of engineering activities

that support the safe operation of Salem Units 1 and 2. This inspection

focused primarily on the activities of the plant technical department. Within

these areas, the inspection consisted of examinations of procedures and

representative records, interviews with personnel, and observations by the

inspector.

The inspectors found that there has been an improvement in communications

between the engineering departments and other plant departments.

In addition,

the inspectors found that some long-standing equipment and system operational

problems are now being addressed.

Although the need for good*root cause analysis is recognized by the

engineering staff, the inspectors found that the quality of problem resolution

was variable.

In some cases, the root cause of problems was not identified

and in other examples unexpected system responses were not fully investigated

and resolved prior to returning equipment to service. Also, the quality of

operability determinations was not consistent in that thorough consideration

and documentation of the implications of technical specification bases and the

accident analyses results was often not included.

The inspectors concluded that there continued to be inconsistencies in the

quality of engineering activities as was discussed in the recently issued

systematic assessment of licensee performance report for the Salem Generating

Stations, Units 1 and 2 (Report Nos. 50-272; 50-311/93-99) .
