NRC INSPECTION NOS.

Dear Mr. Eliason:
The enclosed report documents an inspection for public health and safety,
conducted by Mr. C. Marschall, Senior Resident Inspector and other members of
the NRC staff at the Salem Nuclear Generating Station for the period between
December 18, 1994 and January 28, 1995. These finding were previously
discussed with Mr. J. Summers, General Manger-Salem Operations, and other
members of your staff.
During this period, we noted performance improvement relative to system
engineering inspection activities and timely communication of related concerns
to operations.
We also observed that your Operations organization effectively
contributed to successful completion of complex safety-related maintenance
activities.
We also noted your safety conscious decision to fully resolve the
pressurizer safety relief valve leakage problems affecting Unit 2 before *
returning the unit to service.
We also observed your continued activities to resolve recurring Automatic Test
Insertion (ATI) faults within your Safeguard Equipment Control (SEC) system.
However, it appears that a more thorough root cause approach to previous ATI
test faults may have avoided the operational challenge to Unit 1 operators due
to the recurri.ng SEC operability problem.
Based on the results of this inspection, it appears that you failed to adhere
to the requirements of Technical Specification 3.3.2.1.b relative to
maintaining a minimum of one engineered safety feature actuation steam flow
instrumentation channel operable while Unit 2 was in Mode 3 on December 26,
1994.
However, since this violation was self-identified by your operating
organization, and our inspectors confirmed that you met the criteria for
enforcement discretion as specified in 10CFR2, Appendix C, Section VII, this
violation will not be cited.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice, a copy of
this letter will be pl aced in the NRC Pub l i c Dotument Room.
