NRC Combined Inspection Nos. 50-272/94-20; 50-311/94-20; 50-

354/94-20

Dear Mr. Eliason:

An announced safety inspection of the solid radwaste processing and

radioactive material transportation programs at Salem and Hope Creek

Generating Stations was conducted by

Mr. J. Noggle of this office on August

29 - September 2, 1994, of activities authorized under NRC License Nos. DPR-

70, DPR-75 and NPF-57.

The inspection findings were discussed by Mr. Noggle

with members of your staff on September 2, 1994.

Areas examined during this inspection are important to health and safety and

are discussed in the enclosed inspection report. These areas included a

review of radwaste processing, preparation and shipment of radioactive

material, and onsite storage of radioactive wastes.

The inspection consisted

of observations of onsite activities, interviews with personnel, and selective

examinations of procedures and representative records by the inspector.

Our inspection of your solid radwaste and transportation programs at both

Salem and Hope Creek Generating Stations determined that they were of very

good quality.

Your recent audit of these programs was observed to be weak

because of lack of technical depth, although your overall quality assurance

oversight of the programs was determined to be adequate.

Two events were reviewed during this inspection which involved the release of

contaminated material from the radiologically controlled area.

One of those

events resulted in the contamination of two individuals. The first event,

reported by your staff at Salem Generating Station, involved an operating

procedure weakness in the use of an automatic equipment survey monitor.

This

event proved to be of limited safety significance and was adequately resolved

by your staff, thereby meriting the exercise of enforcement discretion by the

NRC in accordance with 10 CFR 2, Appendix C, Section VII.

The second event,

identified and reported by your staff at Hope Creek Generating Station was

documented in a previous NRC inspection report (No. 50-354/94-13) as an

unresolved item. This event occurred on July 14-15, 1994 and involved the

contamination of two painters, apparently by contact with a contaminated

ladder found outside the radiologically controlled area.

Your staff had

completed its review, but had not determined how the contaminated ladder was

released from the radiologically controlled area. Therefore, while corrective

and preventive actions were implemented, we are unable to determine at this

time whether those actions will be effective to preclude recurrence of this

event. Accordingly, this event does not warrant the exercise of enforcement

discretion and is cited as a violation of 10 CFR 20.1801, failure to secure

licensed material from unauthorized removal from a controlled area. This

violation is cited in the enclosed Appendix A.

You are required to respond to

the enclosed Notice of Violation and should follow the instructions contained

therein.

In your response to the violation, we ask that you reexamine your

investigation as to the cause(s) of the release of material from the

controlled area and provide the results of that review, along with additional

areas of investigation, if any, and any additional corrective and preventive

actions you have taken and plan to take to prevent recurrence.

Please include

your schedule for completion of those items.
