APPENDIX A REVISED NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Nuclear Generating Station Units 1and2 Docket Nos: 50-272 50-311 License Nos: DPR-70 DPR-75 During an NRC inspection conducted on October 17 - November 27, 1993, a violation of NRC requirements was identified and cited previously in NRC correspondence dated January 10, 1994. Upon review of the licensee's subsequent response, dated February 10, 1994, the NRC has revised that violation. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1992), the revised violation is listed below:
Technical Specification 3.8.1.1.b for each unit requires that for Modes 1, 2, 3, and 4, three separate and independent emergency diesel generators (EDGs) shall be operable. Section 9.5.6 of the Updated Final Safety Analysis Report indicates that each EDG is equipped with four air start motors, and that an EDG can be started by any two motors. Operability, as described in the current licensing basis, including Technical Specification 4.8.1.1.2.a.2, requires that each EDG be able to start from ambient condition and accelerate to at least 900 rpm in less than 10 seconds, or otherwise be subject to the requirements of Technical Specification 3.8.1.1, Action
- a. which requires that with one less than the required EDGs operable, PSE&G shall demonstrate the operability of the remaining A.C. sources by performing Surveillance Requirements 4.8.1.1.1.a and 4.8~ 1.1.2.a.2 within one hour and at least once every 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> thereafter.
Contrary to the above, on the following dates, while in one of applicable operating modes, the licensee isolated two of the four air start motors associated with the respective EDGs (i.e., April 9, 1993, the 2C EDG; on May 13, 1993, the lB EDG; on August 5, 1993, the lC EDG; and on September 12, 1993, the 2B EDG), without performing any surveillance to assure that the two remaining air start motors would function*. to start the respective* EDG from ambient**
condition and accelerate to 900 rpm in less than 10 seconds to demonstrate operability or otherwise initiating the requirements of Technical Specification 3. 8.1.1, Action a.
This is a Severity Level IV violation (Supplement I).
Public Service Electric and Gas Company is not required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission relative to this matter, since previous correspondence, dated February 10, 1994, provided sufficient description of the corrective actions taken with regard to this violation.
