INSPECTION REPORT NOS. 50-272/93-29, 50-311/93-29 AND.

50-354/93-30

This refers to the routine announced inspection conducted by Mr. A. Lohmeier of this office

on December 13-17, 1993, at the Salem and.Hope Creek Nuclear Generating Stations in

Hancocks Bridge, New Jersey. Mr. Lohmeier discussed the results of the inspection with

members of your engineering and licensing staff at the conclusion of the inspection. Prior to

this inspection, on December 17, 1993, at the PSE&G Research and Testing Laboratory in

Maplewood, New Jersey, the inspector reviewed the emergency diesel generator (EDG)

cylinder liner failed materials test results.

This inspection was directed at both the engineering activities related to evaluating the root

cause of cylinder liner cracking at the Salem Nueiear Power Generation Station and the

retention of operating cycle transient data for the primary loop components of both Salem

and Hope Creek Nuclear Power Generation Stations.

At the Salem site, Mr. Lohmeier reviewed the scope of the root cause team efforts to provide

a description of the cause of the EDG cylinder liner failure. The effort toward solution of

this problem was extensive and comprehensive. The EDG system plays an important role in

protection of the public health and safety during emergency operating conditions.

At both Salem and Hope Creek and the headquarters engineering department, Mr. Lohmeier

reviewed PSE&G compliance with the requirements of the technical specifications relating to

retention of cyclic operation data. He also reviewed the process of engineering oversight of

the collected cyclic operational data. It was found that the requirement for collection and

retention of cyclic data was satisfied at both Salem and Hope Creek .

It was noted by the inspector that the number of some transients experienCed by primary

system components -will exceed that for which the components were designed over the license

_lifetime. It was also noted, for some primary components, the stress reports and fatigue

evaluations were not available at the site for review by the inspector. Because the inspector

was unable to review these reports, while at the plant site, the inspection remains incomplete.

These document reviews will be tracked as unresolved items URI 354/93030-01 and URI

272/93029-01. We will plan to review the documents during our next inspection.

Your response to the unresolved items will be appreciated. Thank you for your cooperation

in this matter.
