SUBJECT:
SALEM AND HOPE CREEK RESIDENT INSPECTION NOS. 50-272/93-27;
50-311/93-27; 50-354/93-27
The enclosed report documents an inspection to assure public health and safety, conducted by
Mr. C. Marschall, Senior Resident Inspector and other members of the NRC resident and
regional staff at the Salem and Hope Creek Nuclear Generating Stations for the period
between November 27, 1993 and January 15, 1994. The inspectors discussed the findings of
this inspection with Messrs. C. Vondra and R. Hovey of your staff .
Based on the results of this inspection, it appears that one of your activities was not
conducted in full compliance with NRC requirements, as set forth in the attached Notice of
Violation. In accordance with 10 CFR 2. 790 of the NRC's "Rules of Practice", a copy of
this letter and its enclosures will be placed in the NRC Public Document Room. The
responses directed by this letter and the enclosed Notice are not subject to the clearance
procedures of the Office of Management and Budget as required by the Paperwork Reduction
Act of 1980, Public Law No. 96.511.
The violation, involving failure to adequately control trouble-shooting and corrective action
for residual heat removal check valve leakage, appears to be an additional example of the
work control problems previously documented in NRC Inspection Report 50-272/93-23, 50-
311/93-23, and 50-354/93-25. We are concerned that the inadequate control of work
activities has the potential to significantly affect worker safety and the ability to maintain the
plant in a configuration that is within the design basis assumed in the Safety Analysis Report.
We consider that prompt management attention for this concern is warranted to assure timely
corrective action.
