NRC COMBINED INSPECTION REPORT NOS. 50-272/93-82; 50-311/93-82
This refers to the announced electrical distribution system functional inspection (EDSFI:{
conducted by Mr. Leonard Cheung of this office from August 16 to September 3, 1993. The
inspection included a review of activities authorized for Salem Units 1 and 2. At the
conclusion of the inspection, the findings were discussed with those members of your staff
identified in the enclosed report. Additionally, the status of the inspection findings was
discussed with Messrs. M. Burnstein and B. Thomas of your staff during subsequent
telephone conversations on September 29, 1993, and October 27, 1993.
This inspection was directed toward areas important to public health and safety. The purpose
of this inspection was to determine whether the electrical distribution system as designed,
installed, and configured at Salem Units 1 and 2 is capable of performing its intended safety
functions. In addition, this inspection also included a review of your self-assessment of the
electrical distribution system and a review of the August 19, 1993, event involving a battery
cell voltage dropping below the technical specification limit.
The team determined that your self-assessment was adequate, although some weaknesses
were identified as discussed in the attached inspection report. In addition, based on the
team's independent review of selected electrical and mechanical design areas, and
. maintenance and test areas, we determined that, in general, your electrical distribution
system is capable of performing its intended function.
Based upon the results of this inspection, one of your activities appeared to be in violation of
NRC requirements, as indicated in the Notice of Violation enclosed as Appendix A. This
violation involved failure to follow station procedures as a result of your staff's action in
response to the August 19, 1993, event. In addition, two of your activities appeared to
deviate from the Salem updated final safety analysis report, as indicated in the Notice of
Deviation enclosed as Appendix B. One of these deviations pertains to emergency diesel

generator (EDG) equipment protection against a tornado-generated missile. This deviation
applies to Salem Unit 2 only. The other deviation, which relates to EDG fuel supplies, was
originally identified by your self- assessment EDSFI team. Corrective actions for this issue
were not implemented because your staff took credit for the nonsafety-related fuel oil storage
tank. You are required to respond to the Notice of Violation and the Notice of Deviation.
In preparing your response, you* should follow the instructions provided in Appendices A
and B.
In addition, 13 unresolved items and two observations were identified as listed in the
Executive Summary of the inspection report. *We request that you provide to us, in writing,
within 60 days of receipt of this letter, a schedule for the resolution of the 13 unresolved
items.
In accordance with 10 CFR 2. 790 of the NRC's "Rules of Practice," a copy of this letter and
its enclosure will be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosed Notice are not subject to the clearance
procedures of the Office of Management and Budget as required by the Paperwork Reduction
Act of 1980, Pub. L.96-511.
