Dear Mr. Miltenberger:

SUBJECT: COMBINED INSPECTION REPORT NOS. 50-272\\93-80 AND 50-311\\93-80

This letter refers to the announced team inspection conducted by Mr. R. J. Paolino of this

office on May 17-26, 1993, and July 19, 1993, at the Salem Nuclear Generating Station

located at Hancocks Bridge, New Jersey, of activities authorized by NRC License Nos.

DPR-70 and DPR-75 and to the discussions of the team's findings with Mr. S. LaBruna and

other members of your staff at the conclusion of the inspection, and subsequent telephone

conversations between the NRC and PSE&G personnel on May 27, 1993, and June 3, 1993.

The purpose of the inspection was to review your compliance with and efforts for

maintaining the requirements of 10 CFR 50, Appendix R, Sections ill.G, ill.J, and ill.L to

provide the fire protection features for equipment necessary to achieve and maintain the post-

fire safe shutdown capability achieved during the initial validation inspection.

Areas examined during the inspection are identified in this report. The inspection was

directed toward areas important to the public health and safety. Areas covered include the

review and verification of the Fire Protection/Prevention Program and its implementation, the

post-fire alternate shutdown procedure, procedural changes, and modifications with the

potential for affecting the safe shutdown capability. Items covered included fire protection

equipment, penetration seals, fire barriers, water suppression systems, fire extinguishers,

emergency lighting operational functions, procedures, and alternate shutdown capability.

The team's review of the Salem Unit 2 Alternate Shutdown procedures included walkdowns

and comparison of the alternate shutdown methodology with the safety evaluation report

(SER). That review identified several concerns regarding the adequacy of your alternate

shutdown methodology: only one spurious operation was assumed, due to fire in any area,

regardless of the number of unprotected circuits; reliance on repairs (i.e., lifting of leads,

fuse replacement, and installation of jumpers) to provide electrical isolation from the affected

fire area; and lack of a detailed time-line study which demonstrates that required manual

operator actions, including all necessary repairs, can be achieved in a timely manner. Also,

we found differences between your as-installed cable raceway fire barriers and the as-tested

configurations documented in the fire barrier vendor reports. In all, nine unresolved items

were identified (Attachment B). We will inform you of our resolution of these unresolved

items after we complete our internal review.

Based on the team's review of Salem Unit 2 drawings, procedures, surveillance records, and

verification of the installed fire fighting protection equipment, the team concluded that you

have an acceptable routine Fire Protection/Prevention Program that is being properly

implemented and maintained.

Appropriate compensatory measures were in place, as necessary, for the affected cable

raceway fire barriers by the end of the first week inspection.

Within the scope of this inspection, no violations were identified. Your cooperation in this

matter is appreciated.
