SUBJECT:

SALEM AND HOPE CREEK RESIDENT INSPECTION NOS. 50-272/93-20;

50-311/93-20; 50-354/93-20

The enclosed report documents an inspection to assure public health and safety, conducted by

Mr. S. Barr, Acting Senior Resident Inspector, and other members of the NRC resident and

regional staff at the Salem and Hope Creek Nuclear Generating Stations for the period

between July 25, 1993 and September 4, 1993. The inspectors discussed the findings of this

inspection with Messrs. C. Vondra and R. Hovey of your staff.

The inspection report contains the details of an unresolved item opened by the inspectors

which addresses your staff's performance of a 10 CPR 50.59 safety evaluation concerning

the main steam isolation valve sealing system at Hope Creek. The issues involved in this

unresolved item are similar to those of the Notice of Violation contained in NRC Inspection

Report Nos. 50-272/93-08, 50-311/93-08, and 50-354/93-06. These matters were further

discussed in an inspection exit meeting September 16, 1993, between Mr. Ray Brown and

members of your staff and Messrs. John White and Stephen Barr of the NRC staff. From

that meeting, we understand that you are in the process of preparing your response to the

previous Notice of Violation and expect that the response will demonstrate that your future

approach to these issues will fully meet the intent of the requirements specified in 10 CPR

50.59.

Further, w~thin the scope of this inspection, the inspectors noted two instances in which

Technical Specification Action Statements were not acknowledged or met in a timely manner.

These included failure to declare a channel of the Salem Unit 1 Solid State Protection System

(SSPS) inoperable within the required 6 hour6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> period following the results of a surveillance

test; and failure to declare No. 12 cold leg accumulator inoperable following determination

that channel "B" indicated a level of 65 .1 % as compared with the specification of 65 % .

Both issues remain unresolved pending further NRC review. Notwithstanding this

disposition, you should take action to assure that your staff responds appropriately and

promptly to conditions, situations and indications involving deficient system performance.
