SUBJECT:

CLOSEOUT OF CONFIRMATORY ACTION LETfER 1-93-007 Confirmatory Action Letter (CAL) 1-93-007, dated June 8, 1993, identified our understanding of your commitments affecting the operation of Salem Units 1 and 2 relative to the Augmented Inspection Team (AIT) review of Unit 2 Rod Control System anomalies. At the time the CAL was issued, Salem Unit 1 was operating in Mode 1 (Power Operation), and Salem Unit 2 was operating in Mode 3 (Hot Standby). Subsequently, the AIT completed its on-site efforts, and your organization made significant progress in understanding the rod control systems problems and identifying causes and corrective actions. Accordingly, we consider that the commitments are completed, as follows:

1.

Maintain the plant [Salem Unit 2] in a shutdown condition (not to exceed Mode 3) until the AIT has completed all of its on-site inspection and investigation efforts.

Salem Unit 2 continued in Mode 3 while the AIT efforts continued on-site. On June 27, 1993, the AIT completed its on-site inspection and investigation efforts. This item is closed.

2.

Assure the AIT leader is cognizant of, and agrees to, any rod control system equipment operation, testing, or troubleshooting efforts while the AIT is in progress.

This commitment was satisfactorily met. This item is closed.

By June 9, 1993, submit a completed Justification for Continued Operation (JCO) for Salem Unit 1, and verify that reactor operations will be performed in accordance with that JCO.

On June 8, 1993, you submitted Engineering Evaluation S-C-RCS-0819: Justification for Continued Operation SGS Rod Control System. The scope of that JCO applied to Salem Unit 1 operating in Mode 1 with the Rod Control System in manual mode, and Salem 9307070007 930701 PDR ADOCK 05000272 P

Unit 2 operating in a subcritical condition. The JCO did not provide for any restart activities. Later, on June 8, 1993, Salem Unit 1 experienced a turbine trip/reactor trip due to loss of circulating water pumps. Accordingly, reactor operations were constrained by the existing JCO. After extensive discussion with the NRC staff, on June 17, 1993, you submitted a request for emergency license amendment, which included a detailed engineering evaluation and operating commitments to support the amendment and restart of both Salem units. Upon independent verification of your actions by the NRC resident staff and the AIT, and NRC staff review of your submittal, the NRC approved restart of Salem Unit 1 on June 17, 1993. This item is closed.

Upon completion of the AIT on-site efforts, advise me of the results of your assessment and corrective measures, and gain my agreement prior to restart of Salem Unit 2.

In a meeting at our regional office on June 18, 1993, you and your staff presented the results of PSE&G's investigation and assessment of the rod control system problems.

Additionally, you identified corrective actions, taken or planned, to support restart activities on both Salem units. Subsequently, we verified that your staff:

Implemented the specifications of the approved engineering evaluation (as provided in the previously referenced June 17, 1993, submittal) to support safe operations; Established special Standing Orders relative to the conduct of operations affecting the rod control system; Performed rod control system current traces prior to startup of either unit; Performed increased surveillance testing of the rod control system; Acted to assure that procedures were adequate to address abnormal rod movement; Acted to assure that operators were sufficiently trained to recognize and handle situations involving unexpected rod movement; Installed suppression diodes in the rod control systems; Replaced the rod control logic cards in Unit 2; and Established improved practices for rod control system card removal and installation.

Our staff determined that your senior management thoroughly reviewed restart readiness, and verified and validated the successful completion of all applicable corrective actions and restart commitments. Subsequently, in a telephone discussion between you and I on June 27, 1993, I agreed that you had sufficiently analyzed and corrected the rcxt control system problems, and had established sufficient compensatory and corrective measures to restart Unit 2 in conformance with all safety and regulatory requirements. This item is closed.

In accordance with 10 CPR 2. 790 of the NRC's "Rules of Practice," a copy of this letter will be placed in the NRC Public Document Room.
