Dear Mr. Miltenberger:

SUBJECT: INSPECTION REPORT NOS. 50-272/92-16, 50-311/92-16, AND 50-354/92-17

This letter transmits the report of an announced inspection conducted by Mr. Leonard Cheung

and other NRC personnel from October 26 - 30, 1992, at the Salem Units 1 and 2 and Hope

Creek Generating Station in Hancocks Bridge, New Jersey .. Mr. Cheung discussed the

findings of the inspection with Mr. F. Thomson and other members of your staff during the

exit meeting at the generating stations on October 30, 1992.

This inspection was directed toward areas important to public health and safety. Areas

examined during this inspection are described in the NRC Region I inspection report which is

enclosed with this letter. Within these areas, the inspection consisted of selective

examinations of procedures and representative records, interviews with personnel, and

observations by the inspectors.

Based upon the results of this inspection, it appears that one of your activities was in

11 Appendix A. The violation has been categorized by severity level in accordance with the

. { t2f:~~General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2,

"~/'~~p!Jendix C (Enforcement Policy). You are required to respond to the Notice of Violation.

  • It appears that the above violation is attributable to lack of communication between your

corporate engineering (E&PB) and the Salem site engineering. The corporate engineering

nonconformance report, which identified the deficiency, was closed without assuring either:

1) that the safety impact of the deficiency was properly addressed and the deficiency

corrected, or 2) that the nonconformance was recognized by site engineering to followup the

issue. You should review your current procedures to determine if provisions should be

included to prevent repetition of the above problem.

This inspection revealed that: 1) you had a good program for controlling design changes and

modifications, 2) you made good progress in your engineering activity backlog reduction

effort, and 3) you had an aggressive program to pursue resolution of the Hope Creek

hydraulic control unit relining problem.
