INSERVICE INSPECTION ACTIVITIES SIXTH REFUELING OUTAGE SALEM GENERATING STATION UNIT NO. 2 DOCKET NO. 50-311 Public Service Electric & Gas Company (PSE&G) hereby provides in to this letter, a summary of the Inservice Inspections and Examinations to be performed during the sixth refueling outage for Salem Unit No. 2.
The outage was scheduled to begin on January 4, 1992.
Due to the turbine/generator incident on November 9, 1991, the unit outage commenced on November 10, 1991.
A tentative schedule for performance of required examinations is noted on Enclosure 2.,

SUMMARY
SALEM UNIT NO. 2 SIXTH REFUELING OUTAGE The examination summary outlined below will complete those required to satisfy the first inspection interval at Salem Unit No. 2 Except where otherwise noted, non-destructive examinations are performed in accordance with the ASME Boiler and Pressure Vessel Code 1974 Edition (Summer 75 Addenda).
Contract/Station support, tinder the supervision and direction of the Salem ISI Group, will conduct'the following:
A.
Southwest Research Institute will conduct:
- 1.
ISI Manual ultrasonic, liquid penetrant, magnetic particle, and visual examinations of selected piping and/or component welds.
10 year reactor vessel automated ultrasonic
~xaminations including:
o 5% of circumferential welds o
10% of longitudinal welds o
All nozzles from the wall o
Inlet nozzles and butt welds from the bore and inner radius sections In addition, the following reactor vessel automated examinations will be performed:
o 100% coverage of all accessible areas of the reactor vessel welds.

Examination of the outlet nozzles to the ASME Boiler and Pressure Vessel Code 1986 Edition to avoid mid-interval nozzle examination.
o Core barrel indication resolution

PSE&G Site Services ISI Group will conduct:
- 1.
ISI System pressure/leakage tests as required Visual examinations of nuclear class 1, 2, & 3 supports and snubbers ASME Section XI hydrostatic tests
- 2.
10CFR50 Appendix J Local leak rate testing of all Type "B" penetrations and Type "C" valves Third containment integrated leak rate test (Type "A")
- 3.
NUREG 0578 Waste Gas System integrated leak rate test
- 4.
Technical Specifications Functional testing of three hydraulic and eleven mechanical snubbers Local leak rate testing of the steam generator blowdown isolation (GB4) valves
- c.
Westinghouse Electric Corporation under the direction of the Salem ISI Group will conduct:
Technical Specification eddy current examinations (bobbin coil) in 21 and 23 steam generators (random 6% inspection in each)
Additional bobbin coil eddy current examinations of 21 and 23 steam generators to a total of 50% of all tubes (including Technical Specification sample)
MRPC of 50% of the tubes in 21 and 23 steam generators (hot legs only)
Sludge lancing and FOSAR inspection of all steam generators secondary side
