Subject: NRC Inspection Nos. 50-272/91:.26; 50-311/91-26; 50-354/91-19

This letter transmits the findings of the routine, resident safety inspection *conducted by Mr.

T. Johnson and others from September 11, 1991, to October 22, 1991, at the Salem and

Hope Creek Nuclear Generating Stations. The findings were discussed with Mr. C. Vondra

and Mr. J. Hagan of your staff.

Within the scope of this inspection, the inspectors did D:Ot identify any violations requiring

formal response. However, we are concerned with the Salem pressurizer power operated

relief valve failures that appear to have resulted from insufficient vendor guidance,

deficiencies in the procedure used for maintenance and repair, inadequate administrative

controls, and the apparent lack of aggressive management attention. We understand that

your investigation is continuing and that an updated Licensee Event Report will be submitted.

Please keep the residents informed of the progress of your investigation. No reply to this

letter is required.
