Dear Mr. Miltenberger:
Subject: NRC Inspection Nos. 50-272/91-09; 50-311/91-09; 50-354/91-08; and
Tliis letter transmits the findings of the routine, resident safety inspection conducted by Mr.
T. Johnson and others from March 27, 1991 to May 7, 1991, at the Salem and Hope Creek
Nuclear Generating Stations. The findings were discussed with Mr. C. Vondra and Mr. J.
Hagan of your staff.
Based on the results of this inspection, it appears that certain of your activities at Salem Unit
1 were not conducted in full compliance with NRC requirements, as set forth in the Notice of
Violation, enclosed herewith as Appendix A. You are required to respond to this letter, and
in preparing your response, you should follow the instructions in Appendix A. These
violations involve cases of personnel error, lack of attention to detail, and procedure
noncompliance. Several recent similar examples, involving a variety of functional areas, are
detailed in Section 8.1.C. of the enc~osed report. Viewed collectively, these findings cause
us to be concerned regarding an apparent recent decline in procedural compliance and
attention to detail at Sal.em during February through early May 1991. Accordingly, please
also provide your assessment of this matter within 60 days of the date of this letter.
In accordance with 10 CFR 2. 790 of the NRC's "Rules of Practice," a copy of this letter and
its enclosure will be placed in the NRC Public Document Room. The responses directed by
this letter and the enclosed Notice are not subject to the clearance procedures of the Office of
Management and Budget as required by the Paperwork Reduction Act of 1980, Pub. L. No.96-511.
