A routine, unannounced radiological controls inspection was conducted at the Salem Nuclear
Generating Station by. Messrs. R. L. Nimitz and R. H. Lance on March 4 - 8, 1991. The
findings of the inspection were discussed with Mr. S. LaBruna on March 8, 1991.'
Areas reviewed during the inspection are discussed in. the enclosed inspection report. , The**
inspection focused on ra.diological controls for* the Unlt 1 outage. .
.
. . The inspectors identified significant *.improvement in the review and resolution . of
Radiological Occurrence Reports and commendable ALARA planning efforts for steam
generator primary work activities.
However, inspector observations of work activities
identified weaknesses in ALARA practices for certain tasks. These are discussed in the
enclosed report. Although improvements were noted in general industrial safety practices,
some discrepancies were noted. *
Within the scope of this inspection, one apparent vidlation of NRC requirements was
identified. The violation involved two examples of radiation protection personnel failing to
follow requirements specified in radiation work permits and one example of failure to
establish an adequate procedure to provide guidance for monitoring of personnel working
in radiation dose rate gradients.* The violation is discussed iri the enclosed Appendix A,
