50-311/90-81 - Integrated Performance Assessment Team

Inspection

An Integrated Performance Assessment Team (IPAT) inspection was conducted on

May 14-25, 1990, at the Salem Nuclear Generating Station, Hancocks Bridge, New

Jersey.

The inspection was conducted to evaluate a perceived decline in performance

identified in the most recent Systematic Assessment of Licensee Performance

(SALP) report.

The inspection results are documented in the enclosed report.

At the conclusion of the inspection, an exit meeting was held with you and

members of your staff to present the preliminary inspection findings.

The inspection reviewed a number of functional areas with primary emphasis on

evaluating the effectiveness of your recent initiatives, program changes,

corrective actions and management oversight.

In comparison to the performance

level documented in the last SALP report, the team review of Operations

identified apparent improvement.

Engineering and Technical Support performance

remained comparable although weakness was noted in system engineer performance,

which was identified as a strength in the last SALP*report.

Weaker performance

was identified in the Surveillance and Safety Assessment and Quality Verifi-

cation areas.

The team also identified several important safety questions

regarding boric acid transfer pump operability, safety-related electrical cable

separation adequacy, non-code repairs to ASME code systems and residual heat

removal pump interaction.

Several team findings appear to be indicative of a tolerance of degraded

conditions by the site staff.

For example, team walkdowns identified many

deficiencies in the containment buildings after your own staff's walkdowns

identified almost none.

Other findings reflect the need for an improved safety

perspective in key elements of your organization.

For example, weaknesses were

identified in programmatic areas related to the 10 CFR 50.59 safety evaluation

process, surveillance procedure adequacy, corrective action effectiveness, and

procedure change review independence.

Also, several findings of the team indicated weakness in management oversight

of plant activities.

These included the lack of field presence of system

engineers, the failure to review the status of temporary modifications, the

lack of timely implementation of corrective actions, and inadequate control of

overtime.

We will contact you shortly to schedule a management meeting, to discuss your

analysis and evaluation of the inspection results.

At the meeting, you should

be prepared to discuss the key issues highlighted in this letter and you should

present an overview of your plans and processes for developing actions for items

identified as weak or unresolved in the report.

Further, recent SALP reports

and our review of the results of this and other inspections indicate a variation

in performance between the Salem and Hope Creek facilities.

Of particular

interest to us is your assessment of this variation and the reasons for it.

You should also address this concern at the meeting.

The need for followup

meetings or written responses on specific items will be determined subsequently.
