Special Inspection No. fa0-272/89-25; 50-311/89-23

This letter transmits the findings of a special inspection conducted by Mrs.

Kathy Halvey Gibson and Mr. Stephen M. Pindale from November 17-29, 1989, at

the Salem Nuclear Generating Station.

The findings were discussed with Mr.

Lynn K. Miller of your staff .

This inspection reviewed the circumstances surrounding -your identification of a

single failure vulnerability in the Emergency Core Cooling System.

The single

failure vulnerability was introduced when a facility modification was implemented

at both Units 1.and 2 in late 1987. 'As such, the postulated single failure

placed the units in a condition outside of the d~sign bases.

Further, although

you identified and reported this problem, your initial corrective actions did

not appear to full~ and properly resolv~ the concern.

Consequently, we have scheduled an Enforcement Conference at the NRC Region I

office on December 11, 1989 at 10:00 a.m. to disc~ss .this matter.

At the*

conference you should be p~epared to discuss the causes of the event, your

response including corrective actions, and the safety significance, including

any potential broader implications.

Specifically, you should be prepared to

discuss your previous and current programs for reviewing, approving and

  • implementing design changes, and the effectiveness of the onsite review committee.
