Inspection No. 50-272/89-01; 311/89-01

This letter responds to our discussion on April 25, 1989 regarding the March 31,

1989 NRC letter forwarding the results of NRC Inspection 50-272/311/89-01.

I

stated that I intended to amplify the fourth paragraph of the March 31 letter

which requested, in part, additional evaluation of the effectiveness of quality

assurance measures designed to prevent recurrence of nonconformances.

Conse-

quently, we are hereby providing the following 2 paragraphs which should be

substituted for the fourth paragraph of our March 31, 1989 letter.

The second violation involves your failure to correct or prevent recurrence of

self-identified material control nonconformances over an extended period.

We

are concerned that such nonconformances continued during this period despite

recurring quality assurance findings and implementation of escalation procedures

required by the Quality Assurance Program.

Consequently, you should review the

implementation of your quality assurance program in this area and where short-

comings are 1dentified, provide in your response to this letter those corrective

actions necessary to improve the effectiveness of the implementation of the

Program.

We are also concerned that due to your failure to correct the recurring problems,

the number of nonconformances outstanding and the apparent lack of additional

pre-issue controls to verify the appropriateness of materials issued during

this three year period, nonconforming material could potentially have been

installed in safety-related systems.

Please address this concern in your

response indicating what steps you plan to take to determine whether noncon-

forming material has been used in plant systems and, if so, what further action

you plan to take.
