Subject: NRC Combined Inspection Report No. 50-272/88-25; 50-311/88-25
A special radiological controls inspection was conducted by Mr. R. L. Nimitz
during the period November 30, 1988 - January 20, 1989.
The purpose of the
inspection was to review the circumstances and evaluations associated with two
personnel hot particle contamination events which occurred at the Salem Station,
Unit 2 on September 28, 1988 and October 18, 1988, respectively. Also reviewed
were several previously identified NRC concerns.
The findings of the inspection
were discussed with Mr. L. Miller on December 2, 1988 and during a January 26,
1989 telephone discussion .
Within the scope of this inspection, one apparent violation of NRC requirements
was identified and is discussed in the enclosed Appendix A, Notice of Violation.
The violation involves failure to implement radiation protection procedures.
The inspection also identified a number of weaknesses in the oversight of
radiological work activities. This concern was previously discussed with your
staff ( Reference Combined NRC Inspection Report No. 50-272/88-18;
50-311/88-18) .
The results of this inspection and the referenced inspection identified a clear
need to strengthen routine radiological control practices, improve supervisory
oversight and control of work, improve technician and supervisor understanding
of procedure requirements and enhance the quality of procedures.
Your December
22, 1988 response to the previously referenced combined inspection and your
December 27, 1988 response to a subsequent Combined NRC Inspection Report No.
50-272/88-19; 50-311/88-20 indicate efforts are underway to address the concerns
identified. Your efforts in these areas will be reviewed during future
inspections.
You are required to respond to the enclosed Notice of Violation and should
follow the instructions contained therein .
